"It is your hair transplant surgeon who should take a call as to what kind of hairline would fit you. The problem occurs when patients come with their own set of requests by bringing photographs of edited photographs of models they found on social media," says Dr Watts.
The Case for a Mature Hairline
Dr Watts illustrates the point with three real case scenarios of patients aged 20, 27 and 40, showing how a lower hairline that looks fine today can look increasingly irrational and unnatural as each of them ages. Various factors have to be weighed when designing a hairline, including the patient's current age and how a mature hairline would look once they reach a certain age later in life.
A very low hairline chosen at 20 may look striking in the short term, but the same hairline looks distinctly out of place once that patient reaches their late thirties or forties, when the surrounding native hairline has naturally matured. "If a patient does not qualify for a certain hairline design based on the well-established criteria, I usually discourage them," Dr Watts explains.
What Goes Wrong With an Unnatural Lower Hairline
Some clinics will agree to whatever hairline design a patient requests, while more careful, professional surgeons push back on unrealistic requests. According to Dr Watts, the trouble with an artificially low hairline usually surfaces four to five years after surgery, when patients start to feel awkward about it and resort to wearing caps or even consider shaving their heads.
"Many times, lower hairline design exposes gaps of baldness when they start ageing as hair thinning or baldness may occur in some spots. The right approach is to incorporate the features of a mature hairline design given the age of the patient," explains Dr Watts.
The Donor Hair Problem
A lower hairline design covers a larger surface area than a natural one, which means it uses up more grafts. If male pattern baldness advances further later in life, there may be little or no donor hair left on the scalp to correct the appearance, compounding the original design mistake with no easy fix available.
Why a Natural Hairline Lasts a Lifetime
A hairline modelled on a natural, mature pattern is designed to look appropriate at every stage of a patient's life, without ever needing correction. An unnaturally low hairline that suited a patient at 20 looks distinctly odd on the same person at 40 or 45 — and such regret, Dr Watts notes, can even push some patients toward depression.
Every hairline design must look realistic for the whole of a patient's life, not just for a few years after surgery. That is why Dr Watts consistently urges patients to trust their surgeon's clinical judgement over a photograph found online.
