Dandruff is one of the most common scalp conditions, yet many people manage it incorrectly -- sometimes making it worse. In this article based on his educational video, Dr. John Watts -- dermatologist, trichologist and hair transplant surgeon in Hyderabad -- explains the right approach to treating dandruff, from home remedies to medical treatment.

Step 1: Daily head bath and anti-fungal shampoo

"The best solution for dandruff is to take a daily head bath or use an anti-fungal shampoo or lotion to control it," explains Dr. John Watts. If oiling has been applied to the scalp, the oil traps dandruff flakes. These need to be cleared using a ketoconazole shampoo.

Step 2: Overnight ketoconazole lotion

If ketoconazole shampoo alone is not enough, the next step is applying a ketoconazole lotion overnight and washing it off the next morning. This allows longer contact time with the antifungal agent.

Step 3: Consult a dermatologist

"When you consult a dermatologist, the usual line of investigation starts with scalp sampling where a KOH mount is used to diagnose the type of fungal infection on the scalp. Based on one's scalp condition and type of fungal growth, a proper course of treatment is planned," informs Dr. John Watts. For severe cases, treatment may extend over 2 to 3 months using oral antifungal tablets -- ketoconazole, fluconazole, or itraconazole.

The most important thing to avoid: hair oil

"For effective dandruff control, always stay away from hair oil. They can stick together and clog the pores. Dandruff will stay back and lead to bacterial and fungal infections," warns Dr. John Watts. This applies to all oils -- Bhringraj, Almond, and Coconut oil included.

Frequently asked questions

What is the best treatment for dandruff?

Dr. John Watts recommends starting with daily head baths and ketoconazole shampoo. For stubborn dandruff, an overnight ketoconazole lotion can help. Severe cases require oral antifungal tablets under a dermatologist's supervision.

Can hair oil make dandruff worse?

Yes. Dr. Watts warns that hair oils trap dandruff flakes, clog scalp pores, and create conditions for bacterial and fungal infections. Avoid all types of hair oil if you have dandruff.

Can dandruff recur after treatment?

Yes, if your scalp is prone to fungal infections. Re-exposure through shared combs can bring it back. Daily head baths are the best preventive measure.
