Many patients have practical questions about everyday hair care decisions — from post-procedure discomfort to whether combining products is safe. In this article, Dr. John Watts — dermatologist, trichologist and hair transplant surgeon in Hyderabad with over 2,000 successful procedures — answers four common viewer questions.

Why does PRP hurt after the procedure?

Gopi asked Dr. Watts about pain following a PRP session. Dr. Watts explained that the front of the scalp is normally administered with local anaesthesia before PRP as standard practice. However, many doctors and patients skip this step to avoid the minor discomfort of the anaesthetic injection itself.

“Usually, the discomfort accompanied with gradual fading of the numbness effect lasts for 2-3 days only and goes away on its own,” he explained.

If you experience pain after PRP, it is almost certainly because anaesthesia was not used — and the discomfort will resolve in a few days.

Can you apply Tretinoin before Minoxidil?

K Appa Rao asked whether applying Tretinoin cream before Minoxidil improves hair results. Dr. Watts explained that Tretinoin was historically included in Minoxidil formulations to improve skin absorption by drying the skin. However, this combination is no longer recommended.

“Tretinoin is used to help increase the absorption of Minoxidil by drying the skin. However, it is no longer used in combination now as Minoxidil itself causes irritation to some people and if the two are combined and used, it would cause extra irritation, which is not advisable,” he said.

What can be done for hair miniaturisation?

Sai Phani asked about natural or medical remedies for hair miniaturisation caused by androgenetic alopecia. Dr. Watts recommended starting with basic medical options — biotin supplements, Finasteride, or Minoxidil.

“One needs to consistently take the prescribed medication and always follow up with your doctor for consultation. If nothing works, the last option is hair transplant,” he informed.

Do you need to apply hair oil every day?

Vamshi Krishna asked whether daily hair oiling is compulsory. Dr. Watts explained it depends on your hair type.

“If your hair is frizzy and looks dry, you must regularly use hair oil to gain lustre and moisturise the hair. But if you have an oily scalp, you need not apply hair oil,” he said.

For those who want to promote hair growth through oil, he recommends Ayurvedic hair oils.

Frequently asked questions

Why do I feel pain after a PRP procedure?

Pain after PRP is usually because local anaesthesia was not administered beforehand. Dr. Watts says the discomfort from fading numbness lasts only 2-3 days and resolves on its own.

Can I use Tretinoin cream with Minoxidil for better hair growth?

No. Dr. Watts advises against it. While Tretinoin was once combined with Minoxidil to boost absorption, it is no longer recommended as the combination causes excessive scalp irritation.

Is it necessary to apply hair oil every day?

It depends on your hair type. Dr. Watts says those with dry or frizzy hair benefit from regular oiling, but those with an oily scalp do not need it. Ayurvedic oils are recommended for promoting hair growth.
